Forms can be filled out in a customer's profile to include customer surveys, consent forms, liability waivers, and other forms to a customer's profile without requesting the customer to complete it. All of a customer's completed, incomplete, and duplicated forms are available on the Forms tab under the form's name. The date of when the form was completed or last updated is also shown shown beside that version of the form's name.
Unfinished forms can be saved to be completed later, or if a form has already been filled out and submitted, it can be updated as long as an employee has not signed it. Signed forms will need to be duplicated to make changes.
Prerequisites: This feature requires Forms, which is currently available only on the web version of Vagaro, and that you've created at least one form template.
To fill out a form from a customer's profile:

Go to Customers
and select a customer.

Select the Forms tab.

On a phone, swipe left on the top menu to see the Forms tab.

Select the Fill New Form menu, then select the form to be filled out.

In the popup select Fill Form.

Fill in the form, then select Save.

I would like to have my clients fill out their forms in person, how do add more than one form for them to fill out at a time. I want them to be able to fill out their form save and the next one opens for them.
Hi Becky, To do this, you'll need to make these forms mandatory in your form settings. That way, when your customers book their appointments online or come in for their service, all forms will be displayed one after the other.
However, this will also require the customer to fill out the form for online booking through your Vagaro Listing Page. If you don't want this to happen, this setting may not work for you, and you'll need to follow the steps above for each form you want your customers to fill out.
