You can embed a form on your Vagaro MySite for your clients to submit digital forms without booking an appointment. Any forms filled out on your website will be displayed in your Vagaro account when you view responses.
Prerequisites: This feature requires Forms and that you've created at least one form template.
To embed a form:

Go to the Forms page.

Web Version: Select Forms at the top of the screen.

On a Tablet or Pay Desk: Select Forms at the bottom of the screen.

Find the form you want to embed:

Use the Search
bar to look up the template.

In the All Status dropdown, filter for Draft or Published templates.

In the All Types dropdown, filter by Form or SOAP templates.

At the end of the row of the Form to be updated, select the Action
button, then select Embed in Website.

Optionally, enter the Width and Height of your form.

Select Copy Code. A confirmation that the code was copied successfully appears at the top of the screen.

Paste the code into the editor of your site.
