When scheduling or viewing appointments, you can send a form to a customer directly from the Calendar.
Prerequisites: This feature requires Forms and that you've created at least one form template.
To send a form from the Calendar:

Go to the Calendar.

On the Vagaro Pro App: Select Calendar at the bottom of the screen.

On the Web Version: Select Calendar at the top of the screen.

Book an appointment or go to an existing appointment on the Calendar.
Tip
Appointments that require a form have the Form icon.

Select the appointment, then select Forms.

Optionally, if you haven't required a form for this appointment, select the Fill New Form menu, then select a form to be filled out.

The option to either Fill Form or Send to Customer will display.

If you select Fill Form, the form will display on your device to fill out. Select Save to submit the form.
Tip
Expand the Fill New Form menu to select another form.

If you select Send to Customer, have the customer scan the QR code on screen, or enter their email and/or phone number, then select Send to Customer. The customer will recieve an email or text message to fill out the form.

Once the form has been submitted, the Form icon will no longer display on the appointment, and you can view the response from their Customer Profile.
