Sending a form when checking out a customer at Checkout if you'd like to request additional information from the customer after a purchase, such as a customer survey, or if they've forgotten to fill out their pre-appointment form.
Prerequisites: The steps below require at least one form template if you are sending forms to your customers.
To send a form at Checkout:

Go to the Checkout screen.

On the Vagaro Pro App: Select Checkout at the bottom of the screen.

On the Web Version: Select Checkout at the top of the screen.

Select a customer.
Note
Skip to step 6 if you require a form for all customers.

Go to the customer's profile:

Vagaro Pro App: Select the customer's profile photo or icon.

Web Version: Select History.

Select the SOAP or Forms tab.
Note
If you're on the phone, swipe left on the top menu to see the Forms and SOAP tabs.

Select the Fill New Form/SOAP menu, then select the template to be filled out.

For Forms, the option to either Fill Form or Send to Customer will display; otherwise, skip this step.

If you select Fill Form, the form will display on your device to fill out. Select Save to submit the form.
Tip
Expand the Fill New Form menu to select another form.

If you select Send to Customer, have the customer scan the QR code on screen, or enter their email and/or phone number, then select Send to Customer. The customer will recieve an email or text message to fill out the form.

Fill out the SOAP note or form, then continue checking out the customer.

Our system is set up to send customers specific forms depending on the service requested. We have several who receive a notification for all the forms we have in our system and can not skip until they fill each one out. We have double checked that they are not set up for "all customers" but for specific services. Any idea what might be causing this?

Hi,
We are thinking of adding forms to our account. This will be for a membership agreement and a liability waver. Question can we make the form required for each client but only once. I mean that once the client has submitted the forms they will will not be asked again for future appointments. We just need a way to get everyone to sign the forms and keep on file.

Hi Michael, You can require a Form for services, classes, memberships, or every time a customer interacts with your business. See Require a Form to learn more. With this setup, customers will be required to fill out the form of your choosing and will not be required to fill it out again unless they create a new profile.
