Activate the Vagaro Forms Feature – Vagaro Support

The Forms feature lets you create and manage digital forms that clients can complete online or in-house. Choose from Vagaro’s built-in templates, design your own from scratch, or import forms you've created before. You can even require forms to be filled out before a client books an appointment, ensuring you collect all the necessary information ahead of time. Once activated, the Forms feature also gives you the ability to create up to 11 SOAP note templates.

All businesses can activate the Forms feature for an additional cost per month:

$10 USD/month
$15 CAD/month
£10 GBP/month
$15 AUD/month

If you're in the free trial, you can try out this premium feature for the duration of your trial.

Prerequisites: This feature is only available on the web, tablet, Pay Desk, or PayPro version of Vagaro.

Activate Vagaro Forms

To activate the Forms feature:

Go to Forms .
Select Activate Forms.
Select Forms at the top of the screen again to get to the form management screen.

If you're using the tablet, Pay Desk, or PayPro version of Vagaro Pro, select Forms at the bottom of the screen

Deactivate Vagaro Forms

To deactivate Forms:

Go to Settings → Premium Features → Forms.
Select Deactivate Forms → Deactivate.

Important

Disabling the Forms feature does not delete your form responses; however, they will not be visible unless you reactivate the feature.
