Fill Out a SOAP Note From a Customer's Profile – Vagaro Support

Subjective, Objective, Assessment, and Plan notes—commonly called SOAP notes—are used by healthcare and wellness professionals to document a client’s condition and treatment plan.

Vagaro provides three pre-made SOAP note templates to fill out:

SOAP Notes: Create a treatment plan by answering pre-selected questions.
SOAP Notes Face: Create a treatment plan by answering pre-selected questions and referencing treatment areas on a facial anatomy chart.
SOAP Notes Advanced: Create a treatment plan by answering pre-selected questions and referencing treatment areas on an anatomy chart.

Alternatively, create custom SOAP note templates using the Forms feature.

Note

Adding files and photos to a SOAP note requires Vagaro Drive.

Prerequisites: This feature is available for all Vagaro businesses.

To fill out a SOAP note from a customer's profile:

Go to Customers and select a customer.
Select the SOAP tab.

Note

On a phone, swipe left on the top menu to see the SOAP tab.
Select Fill New SOAP, then select the SOAP note to be filled out.
Fill out the SOAP note. For Face and Advanced notes, you can also select the Pencil icon to mark up and take notes on images, then select the checkmark icon to save the changes.
Select Save.
