By default, forms must be manually sent to customers from their profile or directly from the Calendar. If you’d rather have customers fill out a form before they arrive, you can require a form when someone books an appointment, when they purchase a specific membership, or require a form for all new customers. You can make multiple forms mandatory so customers must complete all required forms before booking or checkout.
From your form's settings, you have the option to turn on these settings:

Form is Mandatory for all Clients: Require every customer, new or existing, to complete this form. They'll be prompted to fill out this form when they book or buy anything online (except gift cards), or when you select their profile during checkout, create a new profile for them, send them an invoice, or book an appointment for them. Form is Mandatory When Booking and Form is Mandatory When Purchasing Membership will be hidden after turning on this setting.

Form is Mandatory When Booking: Require customers to fill out a form every time they book a service or class. You can choose to apply this setting to your entire menu or just a few services. You can also require the form to be completed every time a customer books an appointment, even if they've already filled it out before.

Form is Mandatory When Purchasing Membership: Require customers to fill out a form when purchasing a membership. This is especially useful if you need customers to sign a contract for membership or auto-renewal purposes.
Any forms customers submit will be attached to their profile if they're logged in. You can ensure forms aren't submitted anonymously by requiring customer to log in. If the prompt to fill out a form appears when you're booking for the customer or checking out their items, you'll have the option for them to fill out a form from your device, or send it directly to them via email or phone.

From the form designer, in the top-left corner, select Settings.

Turn on the settings you want to configure your form:

To require this form for all customers, turn on Form is Mandatory for All Clients.
Form is Mandatory for All Clients and Form is Mandatory When Booking will not be available if you turn this setting off.

To require this form for customers booking specific services and classes, turn on Form is Mandatory When Booking, then select the services and classes you want to require this form.
If you want to require this form even when your customer has filled it out before, turn on Always Require Form.

To require this form for customers when purchasing a membership, turn on Form is Mandatory When Purchasing Membership, then select the Memberships you want to require this form.
If you want to require this form even when your customer has filled it out before, turn on Always Require Form.

Make changes to your form as needed, then in the top-right corner, select Publish.
