Edit a Form or SOAP Note Template – Vagaro Support

If you need to update your form and SOAP note templates, you can do so on the Forms screen. Updating a template will not affect previous customer form responses.

Prerequisites: This feature require Forms, which is currently available only on the web version of Vagaro.

To edit a form:

Go to Forms .
For the form to be edited, select Action → Edit.
From the form designer, add form elements to your form, or select them to make changes to its settings.
In the top-right corner, select Next to review your form's settings.
Publish your form:
- To save this form as a draft and publish it later, in the top-right corner, select Save as Draft.
  
  This form is saved in form management and can be published at a later time.
- To see a preview of your form and how it'll look for your customers, in the top-right corner, select Preview.
  
  You can select the Computer , Tablet , or Phone icon to see how your form will look on each device.
- To publish your form, go to Settings, then in the top-right corner, select Publish.
  
  Your form is saved in form management.
