You may want to remove unused forms and SOAP note templates to declutter your list of forms and make them easier to find and sort through. Deleting a form or SOAP note template will not remove any submitted responses from your customers' profiles.
Prerequisites: This feature requires Forms, which is currently unavailable on the phone version of Vagaro.
To delete a template:

Go to Forms.

Web Version: Select Forms at the top of the screen.

On a Tablet, Pay Desk, or PayPro: Select Forms at the bottom of the screen.

Find the form or SOAP note template you want to delete:

Use the Search
bar to look up the template.

In the All Status dropdown, filter for Draft or Published templates.

In the All Types dropdown, filter by Form or SOAP templates.

In the row of the form to be removed, select the Action
button, then select Delete.

Select Delete Form or Delete SOAP to confirm.
