Convert Forms and SOAP Notes

On the Forms page, you can convert forms to SOAP notes and vice versa. All of your customer responses are preserved during the conversion process.

Prerequisites: The steps below require the Forms feature, which is currently unavailable on the phone version of Vagaro.

To convert a form or SOAP note:

Go to the Forms page.
- Web Version: Select Forms at the top of the screen.
- On a Tablet or Pay Desk: Select Forms at the bottom of the screen.
Find the form or SOAP note template you want to convert:
- Use the Search bar to look up the template.
- In the All Status dropdown, filter for Draft or Published templates.
- In the All Types dropdown, filter by Form or SOAP templates.
At the end of the row of the form to form or SOAP note to convert, select the icon, then select Convert to SOAP or Convert to Form.
Confirm that you want to convert this form or SOAP note.
