Add the Multiple Choice element to allow customers and employees to answer a question you ask them on your form or SOAP note, up to 200 characters long, by providing them two or more options they can select.
When your customers and employees see the question on the form, they will be able to select one or more of the options you listed.
To add the Multiple Choice element to your form or SOAP note:

To create a form template, in the top-left corner, select → .

To create a SOAP note template, select → .

To edit an existing form, select → .

To duplicate a form, select → .

Under Form Elements, drag the Multiple Choice element into the form designer.
The element's settings will appear on the right side of the screen. If you already have this element in the form designer, select it to make changes.

Add selectable options to this element:

Under Options, enter the name of your first option.

To add more, select Add Option, then enter a name.

To reorder, select Move
and drag it to a new position.

To remove an option, select X.

Customize additional settings:

To align the options, choose Horizontal or Vertical under Align Questions.

To make this element required, turn on Required Field.

To save the customer's selection as a General Tag on their profile, turn on General Tag.

To switch to Choose One or Dropdown, select an option in the Question Type list.

Publish your form:

To save this form as a draft and publish it later, in the top-right corner, select Save as Draft.
This form is saved in form management and can be published at a later time.

To see a preview of your form and how it'll look for your customers, in the top-right corner, select Preview.
You can select the Computer
, Tablet
, or Phone
icon to see how your form will look on each device.

To publish your form, go to Settings, then in the top-right corner, select Publish.
Your form is saved in form management.
