Have customers fill out a form, like a waiver, intake form, or membership contract, before they book or buy. You can require a form for every customer, for specific services and classes, or for specific memberships.
Forms is a premium feature with a monthly fee. If you haven't already, activate the Forms feature first.
To make a form mandatory:
-
Go to Forms
.
-
Create or edit a form:
-
To create a form template, in the top-left corner, select → .
-
To edit an existing form, select → .
-
To duplicate a form, select → .
-
-
From the form designer, in the top-left corner, select Settings.
-
Turn on the settings you want to configure your form:
-
Form is Mandatory for All Clients: Requires this form for all bookings and purchases. When this setting is on, Form is Mandatory When Booking and Form is Mandatory When Purchasing Membership are hidden, because this form is already required for every customer.
-
Form is Mandatory When Booking: Require this form for customers booking specific services and classes.
If you want to require this form even when your customer has filled it out before, turn on Always Require Form.
-
Form is Mandatory When Purchasing Membership: Require this form for customers when purchasing a specific membership.
If you want to require this form even when your customer has filled it out before, turn on Always Require Form.
-
-
Make changes to your form as needed, then in the top-right corner, select Publish.
To stop requiring a form, edit the form, select Settings, turn off the setting, then select Publish.
Here's how each setting works:
-
Form is Mandatory for all Clients: Require every customer, new or existing, to complete this form. They'll be prompted to fill out this form when they book or buy anything online (except gift cards), or when you select their profile during checkout, create a new profile for them, send them an invoice, or book an appointment for them. Form is Mandatory When Booking and Form is Mandatory When Purchasing Membership will be hidden after turning on this setting.
-
Form is Mandatory When Booking: Require customers to fill out a form when they book the services or classes you choose. By default, a customer who has already filled it out isn’t asked again. To ask at every booking, turn on Always Require Form.
-
Form is Mandatory When Purchasing Membership: Require customers to fill out a form when purchasing a membership. This is especially useful if you need customers to sign a contract for membership or auto-renewal purposes.
You can make more than one form mandatory. Customers must complete every required form before they can book or check out.
Note
Each service can have only one required form. To require more than one form for the same service, turn on Form is Mandatory for all Clients instead.
Forms customers submit are attached to their profile if they're logged in. To make sure forms aren't submitted anonymously, turn on Require Vagaro Login to require customers to log in. If the form prompt appears when you're booking for a customer or checking out their items, you can have them fill out the form on your device or send it to their email or phone.
Forms that aren't mandatory can still be sent manually from the customer's profile or from the Calendar.
