Customers typically only need to complete a mandatory form once. However, to ensure the accuracy of your form responses over time, you can set the form to automatically expire after a certain period. After that time, the customer will be prompted to complete and submit the form again. It is best to either always require a form or require a form when booking or purchasing memberships for the most optimal setup.
Note
If you turned on Always Require Form for the Form is Mandatory When Booking or Form is Mandatory When Purchasing Membership settings, the option to set an expiration date will not be available.
Prerequisites: This feature is only available on the web, tablet, Pay Desk, and PayPro versions of Vagaro. Before continuing, ensure you've activated the Forms feature.
To set a form to expire:

Go to Forms

Create or edit a form:

To create a form template, in the top-left corner, select → .

To edit an existing form, select → .

To duplicate a form, select → .

From the form designer, in the top-left corner, select Settings.

Turn on Form Has an Expiration Date. Expires After will appear below this setting.

In Expires After, enter the number of months for when the form will expire. (Maximum of 24 months)

Make changes to your form as needed, then in the top-right corner, select Publish.
