A customer does not need a customer profile to fill out a form; however, if they fill out your form when they're not signed in, it won't be attached their profile at your business. To prevent this from happening, turn on the Require Vagaro login to fill out form setting. That way, when a form is sent to the customer's device, your customer will see the Login Required popup. They can select Login or Sign up now.
The Payment, File Upload, and Contact (when set to update profiles) modules will toggle this setting automatically.
To require login:

Go to Forms

Create or edit a form:

To create a form template, in the top-left corner, select → .

To edit an existing form, select Action
→ .

To duplicate a form, select Action

From the form designer, in the top-left corner, select Settings.

Turn on Require Vagaro Login.

Make changes to your form as needed, then in the top-right corner, select Publish.
