The Signature module allows you to request digital signatures from your customers and employees on your forms and SOAP notes, usually at the end of a survey, waiver, or other form, to obtain consent and acknowledge receipt of information. Before the signature box, you'll also enter a statement or question, up to 200 characters long, so the person understands why they are signing the form.
If you set up the module for an employee signature, you can also select a specific employee who is also required to sign the form. Customers will not see the employee's part of the signature module when completing the form. After an employee has signed a form, it can no longer be modified.
Note
If an employee has signed the form or SOAP note, it can no longer be modified without duplicating it.
To add the Signature module to your form or SOAP notes:

To create a form template, in the top-left corner, select → .

To create a SOAP note template, select → .

To edit an existing form, select → .

To duplicate a form, select → .

Under Form Elements, drag the Signature element into the form designer.
The element's settings will appear on the right side of the screen. If you already have this element in the form designer, select it to make changes.

Enter a Title for this element.

Under Select Signee, choose an option:

To collect a customer's signature, select Customer.

To collect employee signatures, select Employee, then select the employees who have access to sign this form.
Optionally, to allow your employees to add comments while signing, turn on Allow Comments.

Optionally, to make this element required, turn on Required Field.

Publish your form:

To save this form as a draft and publish it later, in the top-right corner, select Save as Draft.
This form is saved in form management and can be published at a later time.

To see a preview of your form and how it'll look for your customers, in the top-right corner, select Preview.
You can select the Computer
, Tablet
, or Phone
icon to see how your form will look on each device.

To publish your form, go to Settings, then in the top-right corner, select Publish.
Your form is saved in form management.
