The Signature module allows you to request digital signatures from your customers and employees on your forms and SOAP notes, usually at the end of a survey, waiver, or other form to obtain consent and acknowledge receipt of information. Before the signature box, you'll also enter a statement or question, up to 200 characters long, so the person understands why they are signing the form.
If you set up the module for an employee signature, you can also select a specific employee who is also required to sign the form. Customers will not see the employee's part of the signature module when completing the form. After an employee has signed a form, it can no longer be modified.
Note
If an employee has signed the form or SOAP note, it can no longer be modified without duplicating it.
Prerequisites: This feature is only available on the web, tablet, or Pay Desk version of Vagaro. Before continuing, ensure you've activated the Forms feature.
To add the Signature module to your form or SOAP notes:

Go to Forms.

Web Version: Select Forms at the top of the screen.

On a Tablet or Pay Desk: Select Forms at the bottom of the screen.

Edit the form or SOAP note that contains the module to be duplicated.

In the left menu, drag and drop the Signature module into the form builder.

Under Select Employee to Sign, choose an option:

Customer: Collect a customer's signature.

Employee: Require an employee or Select Employee to Sign to sign the form. The employees you select will receive a notification to sign this form.

Optionally:

If you selected the Employee signee type, you can toggle Allow Comments to allow your employees to add any additional information under their signature.

To require this module to be completed before submission, toggle the Required switch.

To duplicate the module with the same image, select the Action button, then select Duplicate.

If applicable, make any additional changes, then Save or Publish your form.
