When a customer fills out a form or SOAP note, their response is saved on the Forms screen. You can use this feature to see who has filled out your forms and view these submissions anytime.
Prerequisites: This feature is only available on the web, tablet, or Pay Desk version of Vagaro.
To view all responses:

Go to the Forms page.

Web Version: Select Forms at the top of the screen.

On a Tablet or Pay Desk: Select Forms at the bottom of the screen.

Find the template you want to view:

Use the Search bar to look up the template.

In the All Status dropdown, filter for Draft or Published templates.

In the All Types dropdown, filter by Form or SOAP templates.

Under the Status column, select Published to view responses.

View an individual response.

Select the Individual tab.

Select a customer in the Customer list.

Under History, select a response.

Is there a way to receive the form responses via email to the employee? I'd like to review the responses when they've been submitted and a notification or email sent would be fantastic rather than having to go in and find the info.
Sorry, Danita, but no, it doesn't work that way right now. Another great suggestion, however. Please consider adding a feature request.
Is it possible to reply to the person who submitted the form, or edit the auto response?
Hi Jason... there's no automated response option for a submitted form. You could directly email customers who submit forms when you get the notification. Please consider making this a feature request!
