View a form or SOAP note summary to see an overview of all responses in one place. For example, if you created a customer feedback form, you can use this summary to get a report of what customers think of your business without needing to view each individual response.
Prerequisites: This feature requires Forms and is only available on the web, tablet, or Pay Desk .
To view a summary:

Go to the Forms page.

Web Version: Select Forms at the top of the screen.

On a Tablet or Pay Desk: Select Forms at the bottom of the screen.

Find the template you want to view:

Use the Search
bar to look up the template.

In the All Status dropdown, filter for Draft or Published templates.

In the All Types dropdown, filter by Form or SOAP templates.

Under the Status column, select Published
to view responses.

Select the Summary tab. A summary of all responses to this form or SOAP note will be displayed.
