You can delete duplicated or unnecessary forms and SOAP responses from a customer's profile. However, deleted responses cannot be recovered once they have been removed.
Prerequisites: Your customer must have at least one form or SOAP note response.
To delete a response:

Go to Customers.

On the Vagaro Pro App: Select Customers at the bottom of the screen

On the Web: Select Customers at the top of the screen.

Select the customer.

Select the Forms or SOAP tab.
Note
If you're on the phone, swipe left on the top menu to see the Forms and SOAP tabs.

Select the response you want to delete:
Any forms or SOAP notes the customer has filled out will be displayed below Fill New Form or Fill New SOAP.

Select the form or SOAP note template. All responses for this template will be displayed below.

Select the response. If you aren't sure what response to choose, you can view the submission date next to the customer's name.

Begin deleting the response:

On the Phone: In the top-right corner, select the Action menu, then select Delete.

On the Web, Tablet, or Pay Desk: Select the Delete button.

Confirm that you want to delete the response.
