Ask your customers and employees using the Scale element to provide a rating or measurement by asking a question or requesting up to 200 characters on a form or SOAP note. For example, you can ask them their age range, pain level, or satisfaction of your workplace.
When filling out a form, your customers and employees will be able to rate the answer to your question by selecting a value within a range of numbers, indicating their level of agreement, or assigning a star rating.
