Request your customers and employees to provide their contact information when filling out your form or SOAP note using the Contact module. The contact element can be used to update information in their customer profile or create a new one.
Note
The Contact module cannot be added to mandatory forms.
To add the Contact module to your form or SOAP note:

To create a form template, in the top-left corner, select → .

To create a SOAP note template, select → .

To edit an existing form, select → .

To duplicate a form, select → .

Under Form Elements, drag the Contact element into the form designer.
The element's settings will appear on the right side of the screen. If you already have this element in the form designer, select it to make changes.

Enter a Title for this element.

Customize additional settings:

To stop collecting certain information, such as an email address or phone number, clear the checkbox next to that option.

To use this form to update or create new customer profiles at your business, turn on Update or Save Customer, then choose Create New Customer or Update Existing Customer.
If you choose to update existing customers, your customers will be required to log in to Vagaro. Your customers must also enter their email if they want to create a new profile through the form.

To make this element required, turn on Required Field.

Publish your form:

To save this form as a draft and publish it later, in the top-right corner, select Save as Draft.
This form is saved in form management and can be published at a later time.

To see a preview of your form and how it'll look for your customers, in the top-right corner, select Preview.
You can select the Computer
, Tablet
, or Phone
icon to see how your form will look on each device.

To publish your form, go to Settings, then in the top-right corner, select Publish.

Will the contact field, specifically the referred by field, be automatically uploaded to the client profile to be pulled up in reports, or is this still a manual act on our part?
Hi Nadia,
If you include this module in your form and the customer completes and submits it, the “Referred By” field will automatically populate in their profile.
In any other case, this information will need to be entered manually in the customer’s profile.
