By default, Vagaro has five different SOAP note templates that you can choose from. If there isn't a template you want to use, you can create your own SOAP note templates from scratch.
Note
Unlike forms, SOAP notes have no settings.
Prerequisites: This feature is only available on the web, tablet, or Pay Desk version of Vagaro. Before continuing, ensure you've activated the Forms feature.
To create a SOAP note template:

Go to the Forms page.

Web Version: Select Forms at the top of the screen.

On a Tablet or Pay Desk: Select Forms at the bottom of the screen.

In the top-left corner, Select Create New, then select SOAP.

Select Blank SOAP.

Enter a title for your SOAP note template.

Start designing your SOAP note by drag a module from the side panel then dropping it into the Design tab of the Form Editor.

Preview and publish your form:

To preview your template, select the Computer
or Phone
Preview tabs at the top of the editor.

Select Save for Later to save a draft of your template.

Select Publish to complete your template.
