A SOAP note (which stands for Subjective, Objective, Assessment, and Plan) is a document that's used by healthcare providers to create a patient's chart. It's especially useful for assessing, diagnosing, and treating patients.
SOAP Notes make it easy to capture visit details for each client, retrieve history, and view progress. Additionally, you can duplicate, add to, or edit client SOAP Notes.
Note: This feature only supports PNG, JPG, and PDF files. However, you cannot edit a PDF in either the Vagaro image editor or the SOAP Notes Draw editor.
To add images, files, and photos to a SOAP Note, follow the steps below.

Important: To upload files and images, you must first activate the Vagaro Drive feature (Settings > Vagaro Drive). This allows you to safely add and store data files in Vagaro. Click here to learn more.

Vagaro Pro app
- Tap Customers.
- Select the customer.
- Select the SOAP tab.
- Select Fill New SOAP and pick a SOAP note type (Face, Advanced, or Note,) or select an existing SOAP note to edit.
- Fill out or edit the note. The last entry for the note gives you the opportunity to upload up to 5 images to the note.
- If you upload a single image, it will open in the SOAP Notes Draw image editor, where you can add highlights, arrows, comments, and more before saving the image to your note.
- If you upload multiple images, you can tap Edit for individual images to open in the SOAP Notes Draw image editor and add notations to the image.
- Add notations to the new or selected image as needed.

- Save the SOAP Note.
Web Version
- Navigate to Customers > Customer Management.
- Select a customer.
- Select the SOAP tab.
- Select Fill New SOAP and pick a SOAP note type (Face, Advanced, or Note) or select an existing SOAP note to edit.
- Fill out or edit the note. The last entry for the note gives you the opportunity to upload up to 5 images to the note.
- Upload your image or images (up to 5 files.)
- If you upload a single image, you can add notations as part of the upload process.
- For multiple image uploads, edit each image after uploading to add notations.
- For pictures that are already uploaded, click or tap Edit by an image.
- Add notations to the selected image as needed.
(See Soap Notes Draw.) - Click or tap the checkmark to save your notations.
- Click or tap Save.
Manage Images Saved in SOAP Notes
When you've saved images to a SOAP note, you can view, edit, delete, and download them from within the note.
- Use the Download All link to save copies of all images from the note.
They are saved in one .zip file in a location of your choosing. - Click an image to view it in a gallery format (you can scroll through all images from this view.)
- Use the left and right arrows to scroll through the images.
- Click the Pencil icon to open the image in the Vagaro image editor
Note: This is different from the SOAP Notes Draw editor. - Use the Download button to save just this image.
- Click the Trash icon to delete the image.
- The Detail sidebar opens with the image by default.
- Click the X to close the gallery view.
Edit in SOAP Notes Draw
- To make any changes to the note's content and uploaded images, click Edit at the top of the note.
- Scroll down to the images in the note.
- Click the X for an image to delete it from the note.
- Click Edit to open the image in the SOAP Notes Draw editor and add notations.

- Click Save to save the SOAP Note and all changes to the content and images.
