To require a customer to fill out a form before booking an appointment or a purchase, follow these steps:
1. Select Forms at the top of the screen.
2. Click Create New.
3. Select Blank Form if you're creating the form from scratch, or select one of our Vagaro form templates.
4. Finished designing the form to your liking. Then select Form is Mandatory for All Clients to require a customer to fill out a form before booking a service or a purchase. This option is found at the bottom of the screen.
Select Require Vagaro login to fill out form to require the customer to be logged into their customer account before filling out the form.
5. To require form completion for booking appointments but not mandatory for purchases, select Require Form When Booking. Under Appointments & Classes, click the drop-down to select the classes and services for which you want to require a form when booking the items. This is just a one-time action and only requires the customer to submit the document once.
Select Always Require Form to always require customers to fill out a new form when booking the appointments and classes you selected.
Select Require Form When Purchasing Membership to require the client to submit a form when purchasing a membership.
Select Require Vagaro login to fill out form to require the customer to be logged into their customer account before filling out the form.
Click Publish to make it available to your customers! Click Save for Later if you're still working on it and want to save your progress before completing at a later time.
6. The customer will be prompted to fill out the required form when booking the service or class from the Online Booking Page once the Require Form setting is turned on when they receive the document.
7. If you turned on the Require Vagaro Login option, the customer will see the Login Required popup. The client can click Login to log into their account or select Sign up now to quickly create an account before completing the form.
8. To see if a customer has filled out a required form from the Calendar, go to your calendar. Then find the appointment. A question mark on the appointment block means that the form still needs to be filled out.
9. Click Fill Form to fill out the document for the customer.
