In your form's settings, the option Form is Mandatory When Booking allows you to require a form every time a customer books a service or class at your business. You can configure this setting to only apply to a specific set of services or classes instead of your entire service or class menu, and you can always require this form, even if the customer has filled it out before.
Note
This form will only be required when:

You book or check out an appointment or class session on the Calendar.

A customer books an appointment or class session on your Vagaro Listing Page, Booking Widget, or MySite.
Prerequisites: This feature is only available on the web, tablet, or Pay Desk version of Vagaro. Before continuing, ensure you've activated the Forms feature.
To require clients to fill out a form when booking an appointment:

Go to the Forms page.

Web Version: Select Forms at the top of the screen.

On a Tablet or Pay Desk: Select Forms at the bottom of the screen.

Go to the Form Editor by either creating a new form or editing an existing one.

Scroll down to Form is Mandatory When Booking and toggle it on.
Some additional settings will be displayed below:

Select the box under Appointments & Classes, select the services and/or classes you want to require this form, and then select Add.
You can Select All to require this form for all your services and classes.

Optionally, toggle Always Require Form to always make this form mandatory, even if the customer has already filled it out.

Make any additional changes to your form, then save it.
