To add customer surveys, consent forms, liability waivers, and other customs documents to a customer profile, follow the steps below.
You must first activate the Forms feather and create forms before you can add them to a customer profile. To learn all about forms, see Create a New Form.

Click Customers.

Select the Customer.

Click the green Fill New Form button. Then select the form you want to add to the profile. Remember you first have to create the form on the Forms Management page before you can add it to a client's profile. Click this link for more information: How to Create a Form in Vagaro.

Once you're finished filling out the form, click Save.

Once you've added the form, you'll see it on the left side of the page. Click the date it was submitted to view the form.

Once you select the submitted form, you'll be able to update it. Click Edit to do so.

Once you're finished editing the form, click Save. This action creates a separate submittal date on the left side of the page.

When editing a form, you can also view additional information about the document that was filled out. Click Details.

Once you get to the Details page, you'll be able to see:

The Date and Time the form was submitted.

The device that was used to fill out the form.

The client's IP Address and Location when submitting the document.

When you access a completed form, you can print, export, or remove it. Click the print icon to get a paper copy. Click the icon with the downward arrow to export the form to a PDF. Click the trash icon to remove it.

It may do that already, Ginger. I just did a test: created a new form, made it mandatory for all clients, and had a test customer book an appointment. They had to fill out the form to book and now I see that form in their profile. I also did it with requiring a form for just one service, and you can do the same for a class.
Melissa: There's no way to set a form to be required at a selected date or after a selected period. It might make a good enhancement idea!
You COULD use email/text marketing with a filtered customer list to send a form with a one-time blast. But you'd have to resend it yearly, and then all the customers on the list would get the form at the same time, not necessarily staggered to a year from when they last submitted the form.

Is there a way for forms to be automatically added to customers' profiles once they have been filled out? All of our students are required to fill out a liability waiver and it would be so convenient to be able to quickly check that they have filled it out when taking class!
