To add customer surveys, consent forms, liability waivers, and other forms to a customer's profile, follow the steps below.
Before following the steps below, activate the Forms feature.
The process for Vagaro Pro and Web are the same, so we're only showing Vagaro Pro where there are differences.

Click/tap Customers, select a customer, and then click/tap the Forms tab on their profile.

Click/tap Fill New Form.

Select the form to add.

Click/tap Send to Customer to send the form to your customer to fill out, or Fill Form to fill it out right now.

Fill out the form and click Save (web, tap Submit on Vagaro Pro) to save the form to the customer's profile.
The form is added to the customer's Forms page. From here you can see all the customer's completed forms.

Go to the customer's profile. Select the Forms tab.

Click/tap a form and select a date.
You can see the form as filled out on the selected date.

Click Duplicate (web version, tap Edit on Vagaro Pro) to fill out the form or change the selections. On the web, you'll see the options for sending the form to the customer or filling it out immediately.
Click/tap Save (web) or Submit (Vagaro Pro) to save changes.

To view details of where and when the form was filled, click Details (web).
On Vagaro Pro, tap the
menu and then Details.

To print or download the completed form:

Web: Click the printer icon for a paper copy; click the Download icon to save the form in PDF.

Vagaro Pro: Tap the
menu and then Print.

To delete this copy of the form:

What if i am not getting a option to add form?
