Add the Choice module for customers and employees to answer a question you ask them, up to 200 characters long, in your form or SOAP note by selecting from two or more options you provide.
When your customers and employees see the question on the form, they will have to select one of the options you listed.
Prerequisites: This feature is only available on the web, tablet, or Pay Desk version of Vagaro. Before continuing, ensure you've activated the Forms feature.
To add the Choice module to your form or SOAP note:

Go to the Forms page.

Web Version: Select Forms at the top of the screen.

On a Tablet or Pay Desk: Select Forms at the bottom of the screen.

Create or edit an existing form or SOAP note.

In the left menu, drag and drop the Choose One module into the form builder.

Enter your question in the Ask your question box.

In the Option 1 box, enter the first option to be displayed below the question. Select + Add an option to enter more choices, as required, that can be selected.

If you need to remove any options, select the X button beside the option.

Optionally:

To add the customer's choice to their profile as a General Tag, toggle the General Tag switch at the bottom of the module.

To require this module to completed before submission, toggle the Required switch.

To add an image to this module, select the Action button, select Add Photo, then drag and drop or choose an image from your device.

To duplicate the module with the same image, select the Action button, then select Duplicate.

In the left menu, drag and drop the Choose One module into the form builder.

If applicable, make any additional changes, then Save or Publish your form.
