For businesses that are ready to use the Forms feature, start with a blank template to build a new form from scratch without using Vagaro's templates. Creating a form from scratch lets you put together different kinds of forms for your own business's purposes, such as asking for customer feedback or gathering information before an appointment.
Note
You can't upload your own form templates into Vagaro. All forms must be created in the Forms feature in the business account to be used at the business.
Prerequisites: This feature is only available on the web, tablet, or Pay Desk version of Vagaro. Before continuing, ensure you've activated the Forms feature.
To create a form from scratch:

Go to Forms.

Web Version: Select Forms at the top of the screen.

On a Tablet or Pay Desk: Select Forms at the bottom of the screen.

In the top-left corner, select Create New, then select Form.

Select Blank Form to create a form from scratch.

Enter the name of your form.

Select a module in the left menu and drop it in the form builder.

Scroll to the bottom of your form and configure your form settings:

Preview and publish your form:

To preview your template, select the Computer or Phone Preview tabs at the top of the editor.

Select Save for Later to save a draft of your template.

Select Publish to complete your template.
