Upload Files to SOAP Notes

A SOAP note (Subjective, Objective, Assessment, and Plan) is a document that healthcare providers use to create a patient's chart. It's beneficial for assessing, diagnosing, and treating patients.

SOAP Notes make it easy to capture visit details for each client, retrieve history, and view progress. Additionally, you can duplicate, add to, or easily edit client SOAP Notes.

Note

This feature only supports PNG, JPG, and PDF files. However, you cannot edit a PDF in either the Vagaro Image Editor or the Vagaro Markup Editor.

Prerequisites

SOAP Notes is available to all Vagaro Businesses.

To save files to SOAP Notes, Activate Vagaro Drive.

How do I use this?

Vagaro Pro app

  1. Tap Customers and select a customer.

  2. Select the SOAP tab.

  3. Select Fill New SOAP and pick a SOAP note type (Face, Advanced, or Note,) or select an existing SOAP note to edit.

  4. Fill out or edit the note. The last entry for the note allows you to upload up to 5 images to the note.

    1. If you upload a single image, it will open in the Vagaro Markup Editor, where you can add highlights, arrows, comments, and more before saving the image to your note.

    2. If you upload multiple images, you can tap Edit for individual images to open in the Vagaro Markup Editor and add notations to the image.

  5. Add notations to the new or selected image as needed.

  6. Save your image changes.

  7. Save the SOAP Note.

Web Version

  1. Navigate to Customers → Customer Management.

  2. Select a customer.

  3. Select the SOAP tab.

  4. Select Fill New SOAP and pick a SOAP note type (Face, Advanced, or Note) or select an existing SOAP note to edit.

  5. Fill out or edit the note. The last entry for the note allows you to upload up to 5 images to the note.

  6. Upload your image or images (up to 5 files.)

    1. If you upload a single image, you can add notations as part of the upload process.

    2. For multiple image uploads, edit each image after uploading to add notations.

  7. For pictures that are already uploaded, click or tap Edit by an image.

  8. Add notations to the selected image as needed.

  9. Click or tap the checkmark to save your notations.

  10. Click or tap Save.

Manage Images Saved in SOAP Notes

When you've saved images to a SOAP note, you can view, edit, delete, and download them from within the note.

  1. Use the Download All link to save copies of all images from the note.

    They are saved in one .zip file in a location of your choosing.

  2. Click an image to view it in a gallery format (you can scroll through all images from this view.)

    1. Use the left and right arrows to scroll through the images.

    2. Click the Pencil icon to open the image in the Vagaro Markup Editor.

    3. Use the Download button to save just this image.

    4. Click the Trash icon to delete the image.

    5. The Detail sidebar opens with the image by default.

    6. Click the X to close the gallery view.

Edit in Vagaro Markup Editor

  1. To make any changes to the note's content and uploaded images, click Edit at the top of the note.

  2. Scroll down to the images in the note.

    1. Click X to delete an image from the note.

    2. Click Edit to open the image in the Vagaro Markup Editor and add notations.

  3. Click Save to save the SOAP Note and all changes to the content and images.
